The risk of hematoma is increased if needles are over-inserted into the pterygopalatine fossa beyond the optimum depth of 16 mm; or when needle penetrations are located too posterior to the deposition site on the posterior surface of the maxilla. PSA injections may be contraindicated in patients with clotting disorders or on anticoagulant therapy. Alternate techniques, such as multiple infiltrations may be safer in these situations.
