There are two additional fees a clinical diagnostic lab can charge. What are they and when are they applicable?

specimen-collection fee --charged when a blood sample is drawn through venipuncture/urine is collected through a catheter.
travel allowance  fee-- when a specimen is collected at a nursing home/home-bound patient and travel was necessary to obtain the specimen.
