Describe the Chronic Care Model (CCM)

The Chronic Care Model (CCM) is the first theoretical framework of its kind designed to answer the increasing need for medical care targeting long-term management of heart disease, diabetes, asthma, and other chronic conditions. The CCM was born out of the idea that positive health outcomes are obtainable if a patient is informed and confident in his or her ability to communicate with providers who themselves are accessible and prepared to offer assistance.?
