It is likely that the damage is due to a variety of factors, e.g., synergistic effects of ethanol, acetaldehyde (the first metabolite of ethanol), thiamine deficiency, liver disease, and excessive NMDA receptor activity (which can be excitotoxic) during alcohol withdrawal.
