Necrotizing ulcerative gingivitis has an acute clinical presentation with distinctive features of rapid onset of extreme gingival pain, interdental gingival necrosis, "punched-out" papillae, and bleeding. It is a bacterial infection with associated incidence of stress, increased physical demand, smoking, and decreased nutritional intake. Initial treatment is with analgesics and antibiotics followed by periodontal debridement.
