Contributing factors include flossing and toothbrush trauma. Incorrect flossing can cause "flossing clefts" in which there is a cleft from the gingival margin apically. Teach the patient to hug the tooth with the floss. If he is unable to do so, recommend floss in holder. Incorrect toothbrushing can damage and/or destroy a narrow band of gingiva and result in gingival recession.
