The temperature gradient between the cold modalities and skin temperature is much greater than the gradient between hot modalities and skin temperature. In addition, with heat there is an immediate shunting of cool blood to the area to avoid an extreme increase in tissue temperature. Immediate vasoconstriction occurs with cold so the area of application cannot be rewarmed until there is a need for a cold-induced vasodilatation.
