Answer: Employee ID - The entry field should be gray to indicate the end-user cannot enter data. The data should be automatically generated by the system. The field needs to be sized to reflect the number of characters in the employee ID. For example, if employee IDs are 5 characters, the field size should not be over 5 characters long.

Employee Name - This field should be divided into three separate fields: First Name, Middle Initial, and Last Name. The First Name and Last Name fields are required fields and this should be noted on the form. Add prefix radio buttons for Mr., Miss, or Ms.

Address - This field should be divided in to 4 separate fields: Street Address Line 1, Street Address Line 2, City, and State. The Street Address Line 1 field is required and this should be noted on the form. The City and State fields may be selected from standardized drop-down lists. The preferred method is have have these fields gray to indicate the end-user cannot enter data and automatically populate them based on the zip code. This information is available from the US Post Office.

Zip Code - The field needs to be sized to reflect the number of characters in the zip code.

Phone Number - This field should be divided into two separate fields: Area Code and Phone Number. An additional drop-down field should be added to indicate whether the phone is a cell phone or home phone.
