There is one primary person or group that controls the rest of the population, but the way that they garner such power differs from country to country. There are four primary forms of totalitarianism, and the four types are as follows:
Communist Totalitarianism - This is all about a person.. a revered leader. Think Russia.. or North Korea.
Theocratic Totalitarianism - All about theology.. or a belief system. Think Islam, and the Quran.
Tribal Totalitarianism - Where a particular tribe governs the rest of the population. Africa is the best example these days.
Right-wing Totalitarianism - You can make your own decisions economically (for the most part) but you are forced to conform politically. Think.. Singapore. Or the Phillippines.
