A health record is an ordered set of documents, in the paper context, or a collection of data, in an electronic context, that contains a complete and accurate description of a patient's history, condition, diagnostic and therapeutic treatment, and the results of the treatment.
By contrast, health information refers to the meaningful data relating to the health of an individual that is created or received in any form or medium by a health care provider, health plan, public health authority, employer, life insurer, school or university, or health care clearinghouse.
