Jamal is a 37 year old black male who admittedly has not seen a physician in years. He felt there was no need, as he is physically fit, eats right, and watches his weight. He had to take a physical for a new job and his blood pressure was exceptionally high. So high, in fact, that he was sent to the ED for immediate attention. Fortunately, they were able to get his blood pressure down to

acceptable limits. It was almost miraculous that he did not have a stroke. The blood work done at the time of the physical revealed very high levels of creatinine. He also had proteinuria. He was referred to a urologist for further workup. The workup proved that Jamal has hypertensive chronic kidney disease—Stage 1. He is quite fortunate that the kidney disease was caught in the very early stages. At this point, his hypertension will be managed with ACE inhibitors. He was counseled to maintain his weight, limit his sodium intake and follow a sensible diet. He will be periodically monitored and will return to the office in three months.
