Patients may verbally communicate changes; or they may do so online through a patient portal; or the office may have computer terminals in the waiting area where the patient may edit his/her demographic information; or the staff may turn their screen to the patient and ask him/her to confirm what is there, then verbally confirm or give corrected information.
