S = Subjective: Anything the patient reports but that cannot be observed by the provider; for example, pain, numbness, headache, nausea, etc.
O = Objective: Anything that the provider can observe; e.g., twitching, limp, vital sign readings such as blood pressure, pulse, temperature, respiratory rate, etc.
A = Assessment: The conclusion based on the subjective and objective information; for example, any diagnosis.
P = Plan: What is going to be done to treat the diagnostic findings; for example, any treatments or medications prescribed or plans for further tests, etc.
